The purpose of this form is to submit SLO ASSESSMENT TOPIC STATEMENTS for new courses prior to Curriculum Committee tech review.
Complete and submit the form to receive an emailed receipt, which you should print an attach to the New Course Proposal package that you submit to the Instruction Office.
You will need words to complete one or more (typically 3) assessment topic statements that begin "students completing this course will be able to...".
To make corrections of any kind, simply resubmit this form.
