The purpose of this form is to submit SLO ASSESSMENT RESULTS for TOPICS and experimental courses at the end of the semester. You need to know: (1) the assessment method(s) to be used, (2) the criteria for determining student achievement, (3) the data summary with how many students were included in the assessment and how many achieved (expressed per topic or for the whole), and (4) a plan of action based on your analysis of the results (expressed per topic or for the whole).
