Attention EOPS students! EOPS Progress Reports are required in order to remain in good-standing with EOPS. You must complete your progress reports by the semester deadline in order to receive credit for this requirement.
For your convenience, information regarding the progress report is found on this web page. EOPS students may download the EOPS Progress Report form as well as the Progress Report Memo for completing the EOPS Progress Report. This information is also available in hard copy form in the EOPS Office.

During October (for the fall semester) and March (for the spring semester), meet with each of your instructors during office hours to discuss your progress in the class. (*Note: it may be advisable to complete your progress report prior to the suggested months depending on the content of the course. In most courses, it is advisable to see your instructor after your first test or exam.)
For online classes, you may direct your instructor to this page to download the progress report. Once they have completed the form, ask them to email a scanned copy to you. Alternatively, you may correspond with your instructor via email, and attach that correspondence to the progress report.
Once your progress report has been completed, please bring the completed progress report to your next scheduled EOPS counseling visit no later than Friday, April 6, 2018. You may also submit your progress reports to an EOPS counselor during a drop-in (unscheduled) visit. However, be aware a drop-in visit does not count as one of your two required EOPS counseling appointments.
You cannot drop off your completed progress reports in the EOPS Office. Only an EOPS counselor can review your progress report and give you credit for completing the requirement. Do not leave your progress report at the front desk of the Counseling Center as you will not receive credit.

Note for online instructors: We have created a EOPS Progress Report Form that you may complete and email back to your EOPS student. Conversely, the student may download this form, and email it to you.
