EOPS Term Progress Reports

SPRING 2019 progress reports deadline = Friday, March 29, 2019

Attention EOPS students! The EOPS Progress Reports form and process have changed!

Please plan to complete your progress report requirement in three easy steps:

Step 1: Fill out the top EOPS Student portion of the Progress Report

Step 2: Take the Progress Report form to your instructors’ office hours mid-term. Ask them to complete the bottom Instructor portion of the form. Early March is a good time to start asking for feedback.

Step 3: Once your progress report has been completed, submit your progress report to EOPS using one of two ways:

1) During an EOPS counseling appointment or drop-in with an EOPS counselor by Friday, March 29, 2019
2) In the EOPS Office by Friday, March 29, 2019

** Note: Both EOPS and the Counseling Center close at 3pm on March 29. Please plan ahead. **

Copies of the progress reports are available in the EOPS Office beginning in March. EOPS students may download the EOPS Progress Report form as well as the Progress Report Memo for completing the EOPS Progress Report. This information is also available in hard copy form in the EOPS Office.

When should progress reports be completed?

During October (for the fall term) and March (for the spring term), meet with each of your instructors during office hours to discuss your progress in the class. (*Note:  it may be advisable to complete your progress report prior to the suggested months depending on the content of the course.  In most courses, it is advisable to see your instructor after your first test or exam.)

What if I have an online class?

For online classes, complete the self-reflection part of the Progress Report then email it to your instructor. Once they have completed the form, ask them to email a scanned copy to you. Alternatively, you may correspond with your instructor via email, and attach that correspondence to the progress report.

How do I turn in my completed progress report?

There are now TWO easy ways for you to submit your forms. Once your progress report has been completed, you may submit the form in one of two ways:

1) During an EOPS counseling appointment or drop-in with an EOPS counselor by Friday, March 29, 2019
2) In the EOPS Office by Friday, March 29, 2019

For faculty

The new EOPS progress report process requires EOPS students to first complete a self-reflection of how they believe they are doing in this class. This will be an opportunity for students to ask questions and to also determine whether how a student believes they are doing matches or does not match with their actual progress in the course. Students are being instructed they MUST complete the self-reflection before they provide you with the progress report to complete.
